Get acrobat reader FREE Please complete, sign, and return the application to the credit union along with a copy of a valid state or military photo ID and proof of union membership (a copy of union card or a check stub indicating payment of union dues). If applying for membership as a qualifying family member, please give the name of the current member you are joining under. A copy of a U.S. birth certificate may be used for those under age to obtain a photo ID. Page two of the
printed out application is a disclosure page. A one-time, non-refundable $20.00 dollar entrance fee and an initial deposit of $20.00 (total of $40.00) is required to open an account. The $40.00 may be paid by check, money order, or can be paid through payroll deduction.
