Please complete, sign, and return the application to the credit union along with a copy of a valid state or military photo ID and proof of union membership (a copy of union card or a check stub indicating payment of union dues). If applying for membership as a qualifying family member, please give the name of the current member you are joining under.  A copy of a U.S. birth certificate may be used for those under age to obtain a photo ID.

A one-time, non-refundable $20.00 dollar entrance fee and an initial deposit of $20.00 (total of $40.00) is required to open an account.  The $40.00 may be paid by check, money order, or can be paid through payroll deduction.  If you choose payroll deduction, please click this link, or visit the Services page, go to the Payroll Deduction section, and click on the link to the payroll deduction authorization forms.
